Do you know someone who might benefit from our sub acute care, either short or long term? We would be happy to provide personalized information and answer specific care questions. Choices in health care can be confusing and making a decision can be stressful. Our referral and admission process is simplified, yet thorough, so that resident/patient and family expectations are met to the best of our ability. Any health care professional making a referral can be assured that our staff will respond seven days a week within the hour after receiving the referral. A simple phone call, email or fax to our admissions office or administrator is all it takes.
Our nursing staff will ask for medical history and insurance information. Depending on the complexity of care, a visit to the home or hospital may be recommended to complete an assessment and create the appropriate plan of care. We also encourage families to tour, meet our staff and ask questions. Our staff prides themselves in understanding the person holistically and at the same time remaining timely to accommodate the needs of a quick admission.
